Family NYC Trip – Free Prize Draw Terms & Conditions (“Terms and Conditions”)

1. The promoters of this prize draw (“Prize Draw”) are:
2. • mydentist Group Limited, trading as {my}dentist, a company incorporated in England and Wales with registration number 5657369 and with its registered office address at Europa House, Europa Trading Estate, Stoneclough Road, Kearsley, Manchester M26 1GG (“{my}dentist”); and
   • Element London Limited, a company incorporated in England and Wales with registration number 07073848 and with its registered office at 5 Morie Street, London, SW18 1SL (“Element”)

     - ({my} dentist and Element being referred to as the “Promoters”).

3. The prize is a family trip to New York - prize package for up to 2 adults and 2 children under 16 years old, as further described below (the “Prize”).
4. The Promoters reserve the right to amend the specification of the Prize or offer an alternative prize of equal or greater value, for example if any part of the Prize is unavailable for any reason.
5. No cash alternative to the Prize will be available and the Prize is not transferable or exchangeable.
6. Subject to these Terms and Conditions, there is 1 (one) Prize available to be won by any eligible participant who enters the Prize Draw before it is closed.
7. Participants will be verified for eligibility.
8. Eligible participants will be able to validly enter the Prize Draw from 9.00am on 29^th April 2024 until 5pm on 24^th June 2024 (the “Closing Date”), when the Prize Draw will automatically be closed. Entries received after the Closing Date will be not be valid.
9. The Prize Draw is free to enter and no purchase is necessary.
10. This Prize Draw is open to UK residents over the age of 21 only. It is not open to any employees of (or clinicians engaged by) the Promoters, or any members of their immediate families.
11. Eligible participants must complete all data fields marked with an asterix (*) in the Prize Draw entry form for their entry to be valid.
12. All entries must be submitted via www.mydentist.co.uk/the-garfield-movie by the Closing Date and only one entry per person is permitted.
13. The winner shall be drawn at random draw performed by {my}dentist on or around 25^th June 2024 (the “Draw Date”) from all valid entries received by the Closing Date. The draw will be conducted in accordance with the laws of chance and will be independently supervised.If a non-conforming entry is drawn it shall be disqualified and that ineligible entrant shall not be awarded the Prize.
14. The winner will be notified by {my}dentist within 7 days of the Draw Date.
15. The contact details provided by the winner on entry to the Prize Draw will be shared with Element by {my}dentist.Element will contact the winner to organise receipt of the Prize. If a winner does not respond to Element within 7 days of being notified by Element, then the winner's prize will be forfeited and the Promoters will be entitled to select another winner in accordance with the process described above.
16. The winner may have their name published on the {my}dentist website and in any marketing publications and the winner consents to this when entering the Prize Draw.
17. The Promoters accept no responsibility for entries not successfully completed due to a technical fault including but not limited to technical malfunction, computer hardware or software failure, satellite, network or server failure of any kind.
18. The Prize is supplied by Element. Any personal data submitted by the winner to Element will be treated in accordance with Element’s privacy policy, which is accessible at https://element-london.com/element-london-privacy-policy-travel-english/
19. The Promoters reserve the right to replace the Prize with an alternative prize of equal or higher value if circumstances beyond the Promoters’ control makes it necessary to do so.
20. The decision of the Promoters regarding any aspect of the Prize Draw is final and binding and no correspondence will be entered into about it.
21. The Promoters must either publish or make available information that indicates that a valid award took place. To comply with this obligation the Promoters will send the surname and county of major prize winners and, if applicable copies of their winning entries, to anyone who emails [email protected] or writes to mydentist marketing, Europa House, Europa Trading Estate, Stoneclough Road, Kearsley, Manchester M26 1GG (enclosing a self-addressed envelope) within one month after the Closing Date. If you object to any or all of your surname, county and winning entry being published or made available, please contact the Promoters [email protected]. In such circumstances, the Promoters must still provide the information and winning entry to the Advertising Standards Authority on request.
22. Participants are deemed to have accepted and agreed to be bound by these Terms and Conditions upon entry. The Promoters reserve the right to refuse entry or refuse to award the Prize to anyone in breach of these Terms and Conditions.
23. The Promoters reserve the right to hold void, cancel, suspend, or amend the promotion where it becomes necessary to do so.
24. Insofar as is permitted by law, the Promoter, its agents or distributors will not in any circumstances be responsible or liable to compensate the winner or accept any liability for any loss, damage, personal injury or death occurring as a result of taking up the Prize except where it is caused by the negligence of the Promoters, their agents or distributors or that of their employees. Your statutory rights are not affected.
25. Personal data supplied during the course of this promotion will only be processed as set out in {my}dentist’s Privacy Policy www.mydentist.co.uk/customer-services/patient-privacy. See also paragraph 21 with regard to the announcement of winners.
26. These Terms and Conditions and this promotion are governed by English law. If any entrants to this promotion wish to take court proceedings, then they must do this within the courts in the United Kingdom.

PRIZE DETAILS

Prize package for 2 adults and 2 children under 16 years old includes:
Airfare: Return flights from London, UK to New York, USA
New York Airport Taxi Allowance: Provided to the value of £200 GBP
Accommodation: 3 nights’ luxury (4*) hotel accommodation in New York, based on sharing a quadruple occupancy room
Lunch At Top Lasagne Restaurant: Provided to the value of $200 USD incl. gratuity
Travel Insurance

• Prize is for 2 adults and 2 children under 16 years old;
• The Recipients must travel at the same time with the same itinerary;
• At least one traveller must be aged 21 or over; Anyone under 18 will require parent or guardian permission prior to travel;
• Economy flights from major international airports in London only (inclusive of all applicable airport taxes, fees, and duties) to New York City, USA; Flights may be indirect and not all routings will be available; Flight itinerary will be selected by prize provider at their sole discretion;
• Travel arrangements, including costs relating to public transport, fuel or parking, to and from the winner or companion/s’ home and London departure airport do not form part of the prize and are the responsibility of the winner and their travelling companion/s;
• An airport taxi allowance for use in New York City is provided to the value of £200 GBP and will be sent to the winner via bank transfer to the winner’s nominated accounts, recipient of funds must be 18 or over;
• A valid credit or debit card will be required for security deposit purposes when checking into accommodation;
• Accommodation is based on 2 adults and 2 children under 16 years old sharing one quadruple occupancy room at a 4* hotel in New York City;
• Accommodation will be selected by the prize provider at their sole discretion;
• Accommodation for the Recipients does not include any meals, incidentals, gratuities, or telephone calls. Any other personal expenses incurred during the trip will be the responsibility of the Recipients;
• The prize excludes meals and drinks, excursions, attractions, treatments, visas, personal expenses and anything not expressly stated in the prize package;
• All elements of the prize are subject to availability; If unavailable, experiences and items of a similar nature and value may be offered at prize provider’s discretion; Winners must contact prize provider with three preferred travel dates in three separate months no later than six weeks prior to departure;
• Travel insurance is subject to restrictions and the terms and conditions of the insurance provider. It is the responsibility of the winner and/or their companions to meet any additional costs relating to obtaining insurance cover for any pre-existing medical condition and for any additional costs as may be specified where the winner and/or their companions are aged 70 or over;
• All elements of the prize must be taken at the same time;
• Once booked, the prize cannot be amended or cancelled;
• Prize is non-refundable and non-transferable; No cash alternative is available;
• All travellers will require a passport valid for at least 6 months on date of travel; any necessary visas or insurance are the responsibility of the winner and guests;
• The prize cannot be taken from 21^st December – 4^th January (Christmas and New Year), over UK public holidays, or over Easter Sunday;
• Prize is valid for 12 months from the date of notification of winning, and all travel must be completed within that time;
• Lunch at the specified restaurant is provided to the value of $200 USD including tax and gratuity;
• Prize must be taken in accordance with government guidelines and COVID-19 rules and restrictions in place at the time of travel; The arrangement and additional costs for any COVID-19 related expenses that may be incurred are the sole responsibility of the winner and guest;
• The Prize Provider is Element London Ltd. Any personal data submitted by the winner to the Prize Provider will be treated in accordance with Prize Provider’s privacy policy, which is accessible at https://element-london.com/element-london-privacy-policy-travel-english/
